To schedule an appointment a referral is required. Have your current provider fill out a referral form and fax (417-605-2831) to our office. Our team will review your information and once we determine you are a good candidate, we will schedule you with one of our certified treatment providers for a consultation. During the consultation, a treatment plan will be determined and you will be scheduled for your sessions.
